Pharmacological effect
The drug is intended to treat acne. Isotretinoin is a stereoisomer of the completely trans-retinoic acid known as tretinoin. The mechanism of its action is not fully understood, but it is known to improve the condition in severe acne. This is achieved by suppressing the activity of sebaceous glands and reducing their size, which is confirmed by histological studies.
Lard is the primary growth medium for Propionibacterium acnes bacteria. Decreased production inhibits these bacteria from colonizing the ducts. Aknekutan also inhibits the proliferation of sebum-producing cells and restores the normal process of cell differentiation. It stimulates skin regeneration. In addition, isotretinoin has an anti-inflammatory effect, which additionally contributes to improving the condition of the skin.

Severe forms of acne, including nodular cystic and conglobate forms, and acne, which can lead to scarring
Acne, resistant to other treatments.
Instructions for use and dosage isotretinoin
Aknekutan is taken by mouth, preferably with meals, once or twice a day. Efficacy and possible side effects are dose dependent and may vary from patient to patient, necessitating tailored dosing during treatment.
The initial dose of Aknekutan is 400 mcg/kg/day, which can be increased to 800 mcg/kg in some cases. For severe disease or acne of the trunk, up to 2 mg/kg may be needed.
The optimal cumulative dose is 100 to 120 mg/kg. Usually, complete remission is achieved in 16 to 24 weeks. If dose tolerance is poor, treatment can be continued in lower doses but for longer. In most patients, acne completely disappears after one course of treatment.
If people relapse, they may be treated again at the same dose but no earlier than eight weeks after the first course is completed, because improvement may come later.
In severe renal failure, the starting dose should be reduced to 8 mg/day.
