Benfothiamin is a fat-soluble form of vitamin B1 that is converted to the active coenzymes thiamin diphosphate and thiamin triphosphate in the body. These coenzymes are involved in enzymes such as pyruvate decarboxylase and 2-oxyglutarate dehydrogenase, which play important roles in glucose metabolism.
Pyridoxine (vitamin B6) is converted to the coenzyme pyridoxal phosphate. This coenzyme is involved in the metabolism of amino acids, facilitating their decarboxylation and the formation of physiologically active substances such as epinephrine, serotonin, dopamine and tyramine. Pyridoxal phosphate is also involved in the amino acid transamination process, which is important for anabolic and catabolic processes, as well as for the synthesis and breakdown of amino acids.
Vitamin B6 plays a key role in the metabolism of tryptophan, participating in the four stages of its transformation.

Benfothiamin
Benfothiamin is a fat-soluble derivative of vitamin B1 (thiamin). It has better absorption characteristics than water-soluble thiamin hydrochloride.
Suction
After ingestion, most of the benfothiamin is absorbed in the duodenum, and a minority is absorbed in the upper and middle parts of the small intestine. Suction is by active resorption at low concentrations (≤2 μmol) and passive diffusion at higher concentrations (≥2 μmol).
Allocation
Benfothiamin readily penetrates into various tissues, particularly those where thiamin coenzymes are needed. High levels of coenzymes, such as thiamin diphosphate and thiamin triphosphate, occur in the blood, liver, kidneys, muscles, and brain.
Metabolism
In the intestine, benfothiamin is dephosphorylated by phosphatase enzymes to S-benzoylthiamine. After absorption, enzymatic deenzoylation occurs, resulting in thiamin and biologically active coenzymes such as thiamin diphosphate and thiamin triphosphate.
The drug is intended for oral administration. It is recommended to drink a tablet with sufficient liquid. Adults are usually given one tablet a day, unless prescribed otherwise by the doctor. The dose can be increased to three tablets a day in case of acute need and after consultation with a doctor.
After four weeks of treatment, the doctor evaluates the patient's condition and decides on the further use of the drug. It may be necessary to reduce the dose to 1 tablet per day to reduce the risk of developing vitamin B6-related neuropathy.
