Pharmacological effect
Retinoid to treat severe acne.
Isotretinoin is a stereoisomer of all trans-retinoic acid (tretinoin).
The mechanism of action of Roaccutane is not fully understood, but it is known to improve skin condition in severe acne. It results from a decrease in sebaceous gland activity and a decrease in their size, which is confirmed histologically. The anti-inflammatory effect of isotretinoin on the skin was also established.
In hyperkeratosis of cells in the epithelium of hair follicles and sebaceous glands, the corneocytes exfoliate into the gland ducts. This leads to keratin blockage and excess sebum, which causes comedone formation and sometimes inflammation. Roaccutane inhibits sebocyte division and restores normal cell differentiation, which helps eliminate acne. Because sebum is the main breeding ground for Propionibacterium acnes bacteria, reducing its production reduces the risk of bacterial colonization of the ducts.
