Pharmacological properties:

Pharmakodinamika
Jozamycin is an antimicrobial from the broad-spectrum macrolide group. Active against gram-positive and gram-negative microorganisms, including bond anaerobes. The drug is also active against mycoplasms and chlamydia.

Pharmacokinetics:

After ingestion, the preparation is rapidly absorbed into the GI. The maximum plasma concentration is reached 1 hour after administration. 45 minutes after taking 1 g of the preparation, the average plasma concentration of jazamycin was 2.4 mg/l.

Josamycin penetrates well through biological membranes and accumulates in various tissues and organs: pulmonary, lymphatic, skin and soft tissues, in palatine tonsils, urinary organs. Binding to blood plasma proteins does not exceed 15%. Jozamycin is metabolized in the liver and slowly excreted with bile and in a small amount (≤15%) with urine.

Indications:

Infectious diseases caused by Josamycin-sensitive microorganisms:

ENT-organ infections;
respiratory tract infections;
Dental infections
Skin and soft tissue infections;
infections of the genitourinary system.

The use of Wilprafen is also recommended for allergic reactions to penicillin

application:

The recommended dose for adults and children with a body weight of 40 kg is 1 g (initial); then the drug Josamycin is prescribed 1-2 grams per day (2-4 tablets) in 2-3 reception. In the severe course of disease, the dose can be increased to 3 grams.
The recommended dose for children over 5 years of age with a body weight of ≤40 kg is 40-50 mg/kg of body weight per day, divided into several receptions.
If the drug is not clearly dosed for children, it is used in the form of suspension.
The tablets take without chewing with a small amount of liquid between meals.
The duration of treatment is determined by the doctor. According to WHO's recommendation for antimicrobial use, the duration of treatment for streptococcal infections is at least 10 days.

contraindications:

Increased sensitivity to the components of the drug and other antimicrobial of the macrolide group, pronounced disorders of liver function and bile ducts.

Side effects:

From the gastrointestinal and liver: loss of appetite, nausea, heartburn, vomiting, diarrhea, pseudomembranous colitis, in some cases - increased activity of hepatic transamanaz.
Allergic reactions: rarely - hives.

Special instructions:

In the case of the development of pseudomembranous colitis, the drug should be discontinued and prescribed appropriate therapy.

In patients with impaired liver function and bile ducts treatment is carried out taking into account the results of laboratory studies (with therapy lasting more than 15 days).
Pregnancy and breast-feeding. Although there is currently no data on the embryotoxic action of josamycin, the use of the drug during pregnancy and breast-feeding is permissible in exceptional cases after an assessment of the risk/benefit ratio of therapy.

Use in children. The drug Josamycin can be used in children over the age of 5 years. Children under the age of 5, as well as the impossibility of clear dosing, the drug is prescribed in the form of suspension.

There was no negative impact on the speed of reaction when driving and working with mechanisms.

Interactions:

Since bacteriostatic antimicrobial such as jazamycin and bactericidal antimicrobial can interact with each other, it is necessary to weigh in detail the need for simultaneous use of Wilprafen with these antibiotics.
When used simultaneously with lincomycin, it is possible to reduce the effectiveness of Wilprafen.
After the simultaneous use of Wilprafen and antihistamines that contain terfenadine or astemizole, there may be a slowdown in the excretion of antihistamines, which in turn can cause life-threatening heart rate disorders.

When using Wilprafen together with argot alkaloids, the possibility of developing a pronounced vasoconstrictor effect should be taken into account.
Concurrent administration of Wilprafen and cyclosporin can lead to an increase in blood plasma cyclosporin levels, which in turn can cause impaired kidney function.
With the simultaneous use of Wilprafen and digoxin, it is possible to increase the level of digoxin in the blood plasma.
