Pharmacological effect Ciprofloxacin-Optic
antimicrobial, antibacterial, broad-spectrum bactericidal agent, bacterial eye infections
A broad-spectrum antimicrobial drug, a fluoroquinolone derivative, inhibits bacterial DNA gyrase (topoisomerases II and IV, which are responsible for unwinding DNA to ensure further transcription), disrupts DNA synthesis, growth and division of bacteria. The drug Ciprofloxacin-Optic acts bactericidal on gram-negative organisms during dormancy and division, on gram-positive microorganisms - only during division. Gram-negative aerobic bacteria are sensitive to ciprofloxacin: enterobacteria (Escherichia coli, Salmonella spp., Shigella spp., Citrobacter spp., Klebsiella spp., Enterobacter spp., Proteus mirabilis, Proteus alwards., Morganella morganii., Vibrio., Others.) gram-negative bacteria (Haemophilus spp., Pseudomonas aeruginosa, Moraxella catarrhalis, Aeromonas spp., Pasteurella multocida, Plesiomonas shigelloides, Campylobacter jejuni, Neisseria spp.), some intracellular pathogenic bacteria Brucelamypho trachomatis, Listeria monocytobacterium bacteria: Staphylococcus spp. (Staphylococcus aureus, Staphylococcus haemolyticus, Staphylococcus hominis, Staphylococcus saprophyticus), Streptococcus spp. (Streptococcus pyogenes, Streptococcus agalactiae). Most methicillin-resistant staphylococci are also resistant to ciprofloxacin. The sensitivity of Streptococcus pneumoniae, Enterococcus faecalis, Mycobacterium avium (located intracellularly) is moderate (high concentrations are required to suppress them). Ciprofloxacin-Optic
They are resistant to the action of the drug:
Bacteroides fragilis,
Pseudomonas cepacia,
Pseudomonas maltophilia,
Ureaplasma urealyticum,
Clostridium difficile,
Nocardia asteroides.
Ineffective against Treponema pallidum.
Resistance develops extremely slowly, since, on the one hand, after the action of ciprofloxacin, resistant microorganisms are practically absent, and on the other hand, there are no enzymes in bacterial cells that inactivate it. Ciprofloxacin-Optic
Pharmacokinetics Ciprofloxacin-Optic:
Ciprofloxacin, when applied topically, penetrates well into the tissues of the eye. After a single instillation, its concentration in the moisture of the anterior chamber of the eye after 10 minutes is 0.1 mg / ml. The maximum concentration determined after 1 hour in the moisture of the anterior chamber is 0.19 mg / ml. After 2 hours, the concentration begins to decrease, but its antibacterial effect in the tissues of the cornea lasts up to 6 hours, in the moisture of the anterior chamber up to 4 hours. When applied topically, the half-life from serum is 4-5 hours. Cmax in plasma - less than 5 ng / ml. The average plasma concentration is below 2.5 ng / ml. The drug is excreted through the kidneys mainly (up to 50%) unchanged and up to 10% in the form of metabolites, about 15% is excreted through the intestines, in nursing mothers it is partially excreted in breast milk. After instillation, systemic absorption of the drug Ciprofloxacin-Optic is possible. When treated for 7 days, on average, 4 times a day in both eyes, the average concentration is 2 ng / ml.
Indications Ciprofloxacin-Optic:
Infectious and inflammatory eye diseases (acute and subacute conjunctivitis, blepharitis, blepharoconjunctivitis, keratitis, keratoconjunctivitis, bacterial corneal ulcer, meibomitis, acute dacryocystitis, infectious lesions after eye trauma), pre- and postoperative prophylaxis of infectious diseases.
Contraindications Ciprofloxacin-Optic:
Hypersensitivity to drug Ciprofloxacin-Optic components, viral and fungal lesions of the eyes,
During pregnancy,
lactation period and breastfeeding
children's age up to one year.
Contraindicated in pregnancy, lactation.
Method of administration and dosage Ciprofloxacin-Optic
Locally.
the medicine is instilled in 1-2 drops into the conjunctival sac.
The frequency of instillation depends on the severity of the inflammatory process. In acute bacterial conjunctivitis, simple, scaly and ulcerative blepharitis, meibomite, the drug Ciprofloxacin-Optic is prescribed 4 to 8 times a day, depending on the severity of the disease.
The course of treatment is from five to fourteen days. Ciprofloxacin-Optic
With the disease keratitis:
the drug Ciprofloxacin-Opticis prescribed 1 drop at least 6 times a day, if there is a positive effect, the maximum course of treatment, depending on the degree of corneal lesion, is about 2-4 weeks.
With a bacterial corneal ulcer:
1 drop every 15 minutes for 6 hours, then 1 drop every 30 minutes during waking hours, on day 2 - 1 drop every hour during waking hours, from 3 to 14 days - 1 drop every 4 hours while awake clock. If epithelialization has not occurred after 14 days of therapy, treatment can be continued for 1 week. In acute dacryocystitis, the drug is prescribed 6-12 times a day, 1 drop, the course of treatment should not exceed 14 days.
In case of eye injuries or ingress of foreign bodies and objects into the eyes, for the prevention of infectious complications, the drug is prescribed for 1-2 weeks, 1 drop 4-8 times a day. In the preoperative period: 1 drop 4 times a day two days before the operation and 1 drop every 10 minutes 5 times immediately before the operation.
For the prevention of inflammatory diseases after surgery, the drug is prescribed 4-6 times a day, 1 drop for the entire postoperative period, usually from 5 days to 1 month. for the prevention of infectious complications, the drug Ciprofloxacin-Optic is prescribed for 1-2 weeks, 1 drop 4-8 times a day. Side effects Ciprofloxacin-Optic:
Allergic reactions, itching and burning in the eye, mild soreness and hyperemia of the conjunctiva, nausea, rarely - edema of the eyelids, photophobia, lacrimation, sensation of a foreign body in the eyes, an unpleasant taste in the mouth immediately after instillation, decreased visual acuity, the appearance of a white crystalline precipitate in patients with corneal ulcer, keratitis, keratopathy, corneal infiltration, development of superinfection.
Overdose Ciprofloxacin-Optic:
There are no data on drug overdose. In case of accidental ingestion of the drug, there are no specific symptoms. There may be nausea, vomiting, diarrhea, headache, fainting, anxiety. Treatment: standard emergency measures, sufficient fluid intake, creating an acidic urine reaction to prevent crystalluria.
Interaction with other drugs Ciprofloxacin-Optic
There is evidence that the systemic use of some quinolones leads to an increase in the concentration of theophylline in the blood plasma, enhances the effect of indirect anticoagulants (including warfarin and its derivatives). With the combined use of ciprofloxacin and caffeine, a decrease in the metabolism of caffeine in the liver is observed (a slowdown in its excretion and an increase in concentration in the blood). A temporary increase in serum creatinine concentration has been reported in patients who were prescribed cyclosporine together with the systemic use of ciprofloxacin. Ciprofloxacin-Optic
special instructions Ciprofloxacin-Optic:
The solution in the form of eye drops is not intended for intraocular injection. When using other ophthalmic drugs, the interval between their administration should be at least 15 minutes. Immediately after using the drug Ciprofloxacin-Optic, blurred visual perception is possible, which can lead to difficulties when driving vehicles and when working with mechanisms, it is recommended to start working (driving) 15 minutes after using the drug. During the period of treatment with the drug, it is not recommended to wear contact lenses.
